RESUMO

PURPOSE: To evaluate the influence of bioactive glass and photobiomodulation therapy (PBMT) in calvarial bone repair process in rats submitted to zoledronic acid therapy. METHODS: Twenty-four rats were selected and treated with the dose of 0.035 mg/kg of zoledronic acid every two weeks, totalizing eight weeks, to induce osteonecrosis. After the drug therapy, surgical procedure was performed to create 5-mm diameter parietal bone defects in the calvarial region. The rats were then randomly assigned to groups according to the following treatments: AZC: control group, treated with blood clot; AZBIO: bone defect filled with bioactive glass; AZL: treated with blood clot and submitted to PBMT; and AZBIOL: treated with bioactive glass S53P4 and submitted to PBMT. Tissue samples were collected and submitted to histomorphometric analysis after 14 and 28 days. RESULTS: At 14 days, bone neoformation in the AZBIO (52.15 ± 9.77) and AZBIOL (49.77 ± 13.58) groups presented higher values (p ≤ 0.001) compared to the AZC (23.35 ± 10.15) and AZL groups (23.32 ± 8.75). At 28 days, AZBIO (80.24 ± 5.41)still presented significant higher bone recovery values when compared to AZC (59.59 ± 16.92)and AZL (45.25 ± 5.41) groups (p = 0.048). In the 28-day period, the AZBIOL group didn't show statistically significant difference with the other groups (71.79 ± 29.38). CONCLUSIONS: The bioactive glass is an effective protocol to stimulate bone neoformation in critical defects surgically created in rats with drug induced osteonecrosis, in the studied periods of 14 and 28 days.

RESUMO

It has been shown that 17ß-estradiol (E2) helps to prevent bone loss. This study was undertaken to verify whether E2 action in human osteoblasts involves changes in the transcriptional profile of the TNF-α, IFN-γ, NF-κB, TRAIL, TGF-ß, MMP2, MMP9, RECK, TIMP1, TIMP2, CDK2, CDK4, SRC, RUNX2, and SHH genes. Infraphysiological doses of E2 elevated mRNAs in all genes except for INF-γ, TRAIL, and TGF-ß. Importantly, a significant increase in the CDKs -2 and -4 genes was found, which strongly suggests cell cycle progression, with a potential dependency of Src involvement, as well as a suppression of the osteoblast differentiation machinery, with ECM remodeling being involved. These data suggest that E2 plays an important role in bone formation and remodeling, and Src seems to play a pivotal role in driving cell proliferation and ECM remodeling. Taken together, these findings contribute to an understanding of the effects of infraphysiological E2 on modulating bone homeostasis, favoring bone resorption, and leading to osteoporosis.

RESUMO

Triiodothyronine (T3) and estrogen (E2) play important roles in the bone remodeling process and signaling of receptor activator of the nuclear factor-kappa ß (RANKL) and osteoprotegerin (OPG) expressed by osteoblasts. However, little is known of the molecular action of these hormones in conditions of hyperthyroidism and associated E2 in human cells. AIMS: This study evaluated the effects of the physiological concentration of E2 (10 nM), alone or in association with physiological (1 nM) and supraphysiological (10 nM) concentrations of T3, on RANKL and OPG gene expression in human osteoblasts. MAIN METHODS: Alkaline phosphatase and osteocalcin assays were performed to verify the presence of mature osteoblasts. After mimicking the experimental hyperthyroidism in osteoblasts untreated or treated with E2, RANKL and OPG gene expression was analyzed by real-time PCR and protein expression by western Blot and ELISA. Alizarin Red staining analyzed the amount of bone matrix after hormonal treatments. KEY FINDINGS: E2 enhanced the gene expression of OPG when associated with 1 nM and 10 nM T3. E2 was able to restore the bone matrix after an initial decrease using 1 nM and 10 nM T3. The protective effect of E2 on the RANKL and OPG signaling pathway was demonstrated. E2 restored the bone matrix induced by experimental hyperthyroidism. SIGNIFICANCE: The data highlight the importance of E2 to maintain OPG expression and osteoblast activity against possible loss of bone mass, especially in conditions where T3 is in excess.

RESUMO

OBJECTIVES: To follow healing process of augmented maxillary sinus in rabbits analyzing the histological pattern of bone tissue formation, along with the osteogenic activity and vascularization using a bioactive vitroceramic in comparison to deproteinized bovine bone associated or not with autogenous bone graft. DESIGN: Forty five male adult New Zealand rabbits, 5 months of age, mean weight of 4 Kg, underwent bilateral sinus augmentation surgeries to be divided in five groups: G - (Control) particulate autogenous bone graft (AG), BO - deproteinized bovine bone, BO+G - deproteinized bovine bone + AG, BSi -vitroceramic, and BSi + G - vitroceramic +AG. After 15, 45 and 90 days, all animals were euthanized for specimen's removal to be analyzed under light microscopy, histomorphometry, and immunohistochemistry for Runx2 and VEGF labeling. RESULTS: G, BO and BO+G groups healed uneventfully, allowing the formation of mature remodeling bone at day 90, regarding the association of AG with the biomaterial. On the other hand, BSi and BSi + G groups showed an important cellular reaction and granulation/fibrous tissue formation from the first to the last period of observation. Runx-2 and VEGF immunolabeling were coherent with this result. However, histomorphometry did not reveal significant differences considering new bone formation. CONCLUSIONS: Reconstructed maxillary sinuses using Biosilicate® permitted satisfactory new bone formation in comparison to the deproteinized bovine bone and AG. However, the presence of granulation/fibrous tissue and inflammatory cells associated to the degrading biomaterial indicate that further studies should be careful performed considering the immunological aspect of this new biomaterial.

RESUMO

OBJECTIVE: The aim of the study was to analyze bone matrix (BMX) organization after bone grafting and repair using a new bioactive glass-ceramic (Biosilicate®) associated or not with particulate autogenous bone graft. MATERIAL AND METHODS: Thirty rabbits underwent surgical bilateral parietal defects and divided into groups according to the materials used: (C) control-blood clot, (BG) particulate autogenous bone, (BS) bioactive glass-ceramic, and BG + BS. After 7, 14, and 30 days post-surgery, a fragment of each specimen was fixed in - 80 °C liquid nitrogen for zymographic evaluation, while the remaining was fixed in 10% formalin for histological birefringence analysis. RESULTS: The results of this study demonstrated that matrix organization in experimental groups was significantly improved compared to C considering collagenous organization. Zymographic analysis revealed pro-MMP-2, pro-MMP-9, and active (a)-MMP-2 in all groups, showing gradual decrease of total gelatinolytic activity during the periods. At day 7, BG presented more prominent gelatinolytic activity for pro-MMP-2 and 9 and a-MMP-2, when compared to the other groups. In addition, at day 7, a 53% activation ratio (active form/[active form + latent form]) was evident in C group, 33% in BS group, and 31% in BG group. CONCLUSION: In general, BS allowed the production of a BMX similar to BG, with organized collagen deposition and MMP-2 and MMP-9 disponibility, permitting satisfactory bone remodeling at the late period. CLINICAL RELEVANCE: The evaluation of new bone substitute, with favorable biological properties, opens the possibility for its use as a viable and efficient alternative to autologous bone graft.

Introduction: Periodontal disease (PD) is a chronic multifactorial infection produced by Gram-negative bacteria, which continuously evolves, resulting in a host inflammatory and immune answer to the presence of bacteria and their products which causes bone loss. In chronic disease, there are modifying factors that cause it, but amplify defense mechanisms, such as diabetes, smoking and psychosocial factors. Stress (ST) and Depression (DE) alters the response of the body and leads to a state of immunosuppression. Objective: the objective was to correlate the presence/absence of PD and its extension, with symptoms and signs of ST and DE. Methods: the sample consists in 30 patients with PD and 30 without PD. Periodontal condition was assessed trough evaluations of visible plaque index (PI), probe depth (PD) and clinic insertion level (CIL) and two psychological evaluation instruments were used: the Beck Depression Inventory (BDI) and the Lipp Stress Symptom Inventory (ISSL). Dentistry and Psychological evaluations were conducted in institutional clinical services by students and professionals. Results were correlated by the Spearman test. Results and discussion: the plaque index in patients with PD was 69.23 ± 16.47, while in patients without PD was 12.36 ± 7.25, p> 0.05. Both the correlation of stressing factors with PD (r = 0.1255, p> 0.05) as the correlation of stress symptons with PD (r = 0.2446, p> 0.05) were negative. Conclusion: there was no evidence of relation between the assessed variables.

RESUMO

The aim of this work was to evaluate the role of polylactic /polyglycolic acid copolymer as carrier for BMP-2 on bone regeneration in rat calvarium. Forty five adult male rats underwent 5-mm critical defects in bone calvaria to be divided into three groups according to the filling materials: Control-blood clot; PLGA-polylactic acid Polyglycolic/copolymer; PLGA + BMP-2 -polylactic acid Polyglycolic/copolymer associated with BMP-2. Sacrifice of animals occurred at 5, 15 and 30 days after surgery The evaluation of new bone formation was obtained by histomorphometry, while OPG and RANKL proteins were observed by immunohistochemistry. Statistical analysis was performed by means of nonparametric tests based on quantitative variables of independent samples. Considering the amount of newly formed bone, significant difference was detected between PGLA (178,2±137,5µm) and the other groups, at day 30. In PLGA + BMP-2 and control groups, the expression of RANKL was prevalent on the OPG in the periods of 15 and 30 days, suggesting a favorable condition for bone reabsorption in these periods. Therefore, immunoexpression of RANKL and OPG and bone formation observed in different groups and periods of analysis showed that the polylactic/polyglycolic acid copolymer does not act as a good carrier for BMP-2.

INTRODUCTION: Aging brings a number of health conditions that can compromise the healing process in elderly individuals, significantly when it comes to bone tissue. The aim of the present study was to analyze the effects of zoledronic acid (ZL) therapy on socket healing of aged male rats. MATERIALS AND METHODS: Twenty-four Wistar male rats, 20 months old, underwent surgical procedures for the extraction of the upper right incisor and were divided into two groups according to the treatment: Control (C) - intravenous (IV) 0.9% saline, ZL - 0.035 mg/kg of IV zoledronic acid, both every 15 days. At the fifth dose of both substances, tooth extractions were performed and the animals were euthanized after 14 and 28 days. RESULTS: IV administration of ZL caused OPG-RANKL system imbalance, resulting in deficient bone formation and remodeling and alteration of osteoclast morphology, as well as maintaining persistent inflammation during the healing period. CONCLUSIONS: IV administration of ZL delayed extracted dental socket healing of aged rats, but not enough to cause osteonecrosis, raising a question about different responses to IV BP therapy considering animal age.

RESUMO

OBJECTIVES: To evaluate the effects of green tea intake and zoledronic acid intravenous therapy on teeth socket repair. DESIGN: Sixty male albinus Wistar rats were divided into 4 groups: C-Control, intravenous (IV) 0.9% saline solution (SS), GT-1% green tea in drinking water and IV SS, BP-IV zoledronic acid (BP), and BP+GT-IV BP and 1% green tea. 0.035mg/kg of BP was administered every two weeks. After ten weeks, right upper molars were extracted and the green tea started to be offered for GT and BP+GT. After 7, 14, and 28days the animals were euthanized. RESULTS: Histopathology analysis revealed lack of socket repair in BP and BP+GT groups, which presented significant increased number of polimorphonuclear leukocytes at day 28, in comparison with C (p<0.05). No significant differences were detected between C and the experimental groups at the same period (p<0.05) when considering mononuclear leukocytes. Immunolabeling revealed that the association of BP and GT caused a slight disturbance in OPG/RANKL system and retarded Runx-2 labeling. Although strong TRAP labeling was observed, most of the positive cells in BP and BP+GT groups were not located on bone surface. CONCLUSIONS: Socket healing of rats treated with BP and regular drinking green tea presented no relevant differences in comparison to those treated with BP alone.

The aim of this paper is to describe a clinical case of transoperative bleeding in the anterior mandibular region. A 77 years-old male patient having arterial hypertension was scheduled for dental implant placement. After the osteotomy for implant preparation, abundant bleeding was observed at the fl oor of the mouth, with severe tongue swelling and an increase on blood pressure. The basic management consisted of anti-hypertensive and anxiolytic medication, with two lateral incisions to drain blood, vital signs monitoring. The edema subsided and no airway obstruction or speech problems were observed thereafter. The patient was discarded with appropriate recommendations, and the implants were not installed in the same surgical consultation. There is a considerable risk of bleeding in the anterior mandibular region. However, patients with systemic diseases or under anti-coagulation therapy are more prone. Fenestration of the lingual cortical plate could be a possible reason for profuse bleeding. The clinician must know how to manage these situations to save the patient since local anatomic changes can favor the hemorrhagic accident.

RESUMO

AIM: Oral hygiene technique is an important factor in maintaining the health and comfort of hospitalized patients given the frequent presence of oral biofilm and pathogens brought on by mouth breathing. This is an important practice to assist patients in intensive care, in particular those who are intu-bated and under mechanical ventilation because the realization of oral hygiene reduces the patient's risk of complications and length of hospitalization. The objective of this research was to evaluate the oral health condition of patients hospitalized in an intensive care unit (ICU) and to clarify the importance of protocol standardization involving these patients' buccal hygiene. MATERIALS AND METHODS: In this study, the sample consisted of 45 patients admitted to an ICU who were evaluated in relation to the oral biofilm score index. RESULTS: The results indicated that there was no significant difference in the biofilm score associated with the genre (p = 0.091), age group (p = 0.549), or teething profile (p = 0.207). However, the biofilm score was greater in partial and fully edentulous patients when compared with dentulous patients. CONCLUSION: Based on these results, it is recommended that care providers in ICUs complete the relevant oral health care training programs. CLINICAL SIGNIFICANCE: When in the ICU, suitable dental conduct following a protocol of prevention of oral biofilm can lead to earlier diagnosis and can prevent the spread of pathogenic microorganisms, particularly those that are systemic in patients with low immunity.

RESUMO

Introduction: mechanical hygiene is still the best method forthe maintenance of oral health and prevention of biofilm formation. However, mouthrinses have been increasingly used as an adjunct support especially for post-operatory conditions or when periodontal conditions require. In this way, a number of mouthrinses are available, presenting different active ingredients with predominant antimicrobial action. Objectives: the aim of the present study was to evaluate the topical action of a green-tea extract-based mouthwash on gingival tissue. Materials and Methods: a longitudinal double-blind comparison was performed with two groups of patients clinically evaluated and diagnosed as suffering from gingivitis (n= 40). The patients were divided into two groups, according to the mouthrinse used: GT - green tea (concentration of 20mg/ml), and P - placebo, 0.9% saline solution. Clinical examination of the mucosa, tooth staining, plaque and gingival indices were peformed before and 15 days after the continuous use of the products twice a day. Results: no mucosal or tasting alterations were found, neither tooth staining regardless the mouthrinse used. Statistical assessment did not detect differences on gingival indexes between the groups before and after mouthrinses use; however, plaque indexes were significant lower in patients of GT group, compared to the P group. Conclusions: the use of phytotherapic products with active ingredients should be investigated for biofilm control, responsible for the onset and progression of periodontal disease, as well as other various oral pathologies. Considering the period of evaluation pro-posed in this study, relevant decrease in initial phase of plaque for-mation could be observed with the green tea mouthrinse. Further long-term evaluation studies should be carried-out in order to eluci-date its continuous effects on oral structures.

Introduction: necrotic root canals and untreated are characterized by the presence of a mixed and polymicrobial flora. E. faecalis show high resistance to drugs used during endodontic treatment, one of the few microorganisms which have shown in vitro to resist the antibacterial effect of calcium hydroxide. Furthermore, it is the bacterial species most often isolated, with a prevalence ranging between 29% and 77%, in root canals of teeth with endodontic failure. Objective: the aim of this study was to evaluate the antibacterial activity of the sealers MTA Fillapex, Sealapex, Sealer 26 and Endofill, pure and loaded with amoxicillin and ciprofloxacin against two ATCC strains of Enterococcus faecalis (ATCC 29212 and ATCC 4083). Method: the tests were conducted by means of radial diffusion technique of the cement applied in to the agar, impregnated paper discs applied on the agar and by the direct contact test. For the statistical analysis, the ANOVA and the Tukey?s test were used at a 5% significance level. Results and Discussion: results showed that, through the technique of radial diffusion, regarding the pure cements, the Sealapex was the only one that showed inhibition halo for the two strains. However, considering the association of amoxicillin and ciprofloxacin Fillapex, the MTA showed better inhibition zones for the two strains. All cements had their actions enhanced by with addition of antibiotics (p <0.05). Through the technique of diffusion of cements impregnated in paper discs, the Sealapex pure fresh cement revealed an antimicrobial activity and also in all the variables of time. When loaded with antibiotics, all cements had the antimicrobial action potentiated by drugs in all variables of time. The same result was observed in the technique of direct contact of cement with both strains. Conclusionn: the antibacterial activity against the E. faecalis, of the cements MTA Fillapex, Endofill, Sealer and Sealapex loaded with the antibiotics amoxicillin and ciprofloxacin, is enhanced even after a long period of use of cements.

OBJECTIVE: The aim of the present study was to evaluate, comparatively, the periodontal response during orthodontic treatment performed with self-ligating and conventional brackets. METHODS: Sixteen Caucasian individuals of both sexes, aged between 12 and 16 years old and in permanent dentition were selected. Eight individuals were treated with conventional brackets installed on the lower dental arch and self-ligating brackets on the upper arch. Another eight individuals received self-ligating brackets in the lower arch and conventional brackets in the upper arch. The subjects received material and instructions for oral hygiene. Visible plaque index (VPI), gingival bleeding index (GBI) and clinical attachment level (CAL) were evaluated just after installation of orthodontic appliances, and 30, 60 and 180 days later. Mann-Whitney test was used to compare differences between groups (self-ligating and conventional), two-way ANOVA followed by Tukey's test was used to assess CAL at each site of each tooth. Significance level was set at 5%. RESULTS: No significant changes were found with regard to the assessed parameters (VPI, GBI and CAL) in either one of the systems. CONCLUSION: No significant changes were found with regard to the periodontal response to orthodontic treatment for the variables assessed and between subjects receiving passive self-ligating and conventional brackets. All individuals had received oral hygiene instructions and had their periodontal conditions monitored.
